The IU School of Medicine is inviting physician offices to participate in the testing of an AHRQ questionnaire designed to evaluate the cultures of safety in physician practices. Outcomes include developing a baseline for future comparison, uncovering potential barriers to completing the survey, and understanding perceived value of the survey data for quality improvement initiatives.

The survey will take 10-15 minutes to complete and lunch will be provided for all participants. Each office will receive a summary report of their responses. Confidentiality is ensured.
